When I open the image detail dialog box, the thumbnail shown is of a totally different image. If I try editing any forms (ie: alt text, caption) and saving, the original image will be replaced by the one shown in the thumbnail. This occurs for many of my images (but definitely not all). If I open and close, and open the dialog box again most of these will show the correct image, but for a select few reopening does not fix the problem.
Haven’t seen this on any of our hosted Pressbooks networks. It sounds like a possible database issue to me – as though the book is somehow pointing to incorrect media elements. Can you share the contents of your diagnostics page here for others to see if they notice anything that might be surprising in there? You can get to it by clicking the ‘diagnostics’ link seen in the footer of your admin dashboard when logged in as a Pressbooks user:
