EPUB table of contents problem - not displaying double quotation marks properly

Table of Contents for the epub output does not display double quotation marks properly, instead giving the code &quot You can see it here, if you scroll down.

However, these display fine in the title of the relevant chapter - as you can see on the attached.

Only happens with Calibre e-reader, so not a problem to do with Pressbooks.
